- Navigate to the 'Campus Connection Guest page'
- Click on the 'NDUS eForms' tile (as shown below).
- Click on 'North Dakota State University' and then 'Reactivation Petition' from the left side menu.
- Complete the form and monitor the email address entered on the submission for any communication about the completion of the form.
